Profiles and Accolades

Hospital at Home has been profiled and featured by a number of public and private organizations seeking to improve health care for all Americans.

Health Workforce Solutions (HWS) identified Hospital at Home as one of 24 innovative care models that could be widely replicated throughout the United States. With funding from the Robert Wood Johnson Foundation, HWS researched and identified nearly 200 programs and then selected 60 of them for in-depth research interviews. HWS also convened groups of nurse executives, nurse academics, and health-care executives to discuss criteria for ranking care-delivery models and strategies to encourage model replication. Using this criterion, HWS winnowed the group to 24 innovative care delivery models. Complete profiles of each model are published on the website. The Innovative Care Model project and Hospital at Home were also featured in Health Affairs Grant Watch.

The Agency for Healthcare Research and Quality (AHRQ) profiled Hospital at Home as an AHRQ Health Care Innovation. AHRQ is the nation's lead federal agency for research on health care quality, costs, outcomes, and patient safety. AHRQ developed the Health Care Innovations Exchange as a Web-based resource to help health care professionals share and adopt innovations that improve patient care. The American Association of Homes and Services for the Aging recommended Hospital at Home in its report In the Place They Call Home: Expanding Consumer Choices Through Home and Community-Based Services. The report was issued by the AAHSA's Home and Community Based Development Cabinet to provide members with recommendations on how to create, improve, and expand home- and community-based services.
